ABSTRACT

Abstract Introduction Microsurgery of the ear requires complete evaluation of middle ear surgical anatomy, especially the posterior tympanic cavity anatomy. Preoperative assessment of the middle ear cavity is limited by the permeability of eardrum and temporal bone density. Therefore, middle ear exploration is an extremely useful method to identify structural abnormalities and anatomical variations. Objective The aim of this study is to determine anatomic variations of the middle ear in an autopsy series. Methods All evaluations were performed in the Forensic Medicine Institute Morgue Department. The cases over 18 years of age, with no temporal bone trauma and history of otologic surgery included in this study. Results One hundred and two cadavers were included in the study. The mean age was 49.08 ± 17.76 years. Anterior wall prominence of the external auditory canal was present in 27 of all cadavers (26.4%). The tympanic membrane was normal in 192 ears (94%) while several eardrum pathologies were detected in 12 ears (6%). Agenesis of the pyramidal eminence and stapedial tendon was found in 3 ears. While the ponticulus was bony ridge-shaped in 156 of 204 ears (76.4%), it was bridge-shaped in 25 ears (12.3%). The ponticulus was absent in 23 ears (11.3%). While complete subiculum was present in 136 of all ears (66.7%), incomplete subiculum was present in 21 ears (10.3%). Subiculum was absent in 47 ears (23%). Facial dehiscence was found in 32 ears and the round window niche was covered by a pseudomembrane in 85 ears (41.6%). A fixed footplate was present in 7.4% of all ears, and no persistent stapedial artery was seen in any cases. Conclusion The pseudomembrane frequency covering the round window niche was found different from reports in the literature. In addition, the frequency of the external auditory canal wall prominence has been reported for the first time.


Resumo Introdução A otomicrocirurgia requer avaliação completa da anatomia cirúrgica da orelha média, especialmente da anatomia da cavidade timpânica posterior. A avaliação pré-operatória da cavidade timpânica é limitada pela permeabilidade do tímpano e densidade do osso temporal. Portanto, a exploração da orelha média é um método extremamente útil para identificar anormalidades estruturais e variações anatômicas. Objetivo Determinar as variações anatômicas da orelha média em uma série de autópsias. Método Todas as avaliações foram realizadas no necrotério do Instituto Médico-Legal. Os casos com mais de 18 anos, sem trauma do osso temporal e história de cirurgia otológica foram incluídos neste estudo. Resultados Cento e dois cadáveres foram incluídos no estudo. A média de idade foi de 49,08 ± 17,76 anos. A proeminência da parede anterior do conduto auditivo externo estava presente em 27 de todos os cadáveres (26,4%). A membrana timpânica era normal em 192 orelhas (94%), enquanto várias alterações do tímpano foram detectadas em 12 orelhas (6%). Agenesia da eminência piramidal e do tendão do estapédio foi encontrada em 3 orelhas. Enquanto o pontículo tinha formato de crista óssea em 156 das 204 orelhas (76,4%), tinha o formato de ponte em 25 orelhas (12,3%). O pontículo estava ausente em 23 orelhas (11,3%). Enquanto o subículo completo estava presente em 136 de todas as orelhas (66,7%), encontrava-se incompleto em 21 orelhas (10,3%). O subículo estava ausente em 47 orelhas (23%). Deiscência facial foi encontrada em 32 orelhas e o nicho da janela redonda estava coberto por uma pseudomembrana em 85 orelhas (41,6%). A platina fixa foi observada em 7,4% de todas as orelhas e a artéria estapediana persistente não foi vista. Conclusão A frequência da pseudomembrana que cobre o nicho da janela redonda foi diferente daquela encontrada na literatura. Além disso, a frequência da proeminência da parede do canal auditivo externo foi relatada pela primeira vez.

ABSTRACT

Defeitos parciais de orelha podem ser tratados de diversas formas, dentre elas o fechamento primário, cicatrização por segunda intenção ou retalhos. Diversas opções técnicas foram descritas para a sua reconstrução de modo a manter o contorno natural da orelha, sem sacrificar tecido sadio ou alterar sua estética e função. Apresentamos neste artigo dois casos atendidos no Instituto do Câncer do Hospital de Base de São José do Rio Preto de reconstrução de defeitos condrocutâneos de orelha após ressecção de carcinoma basocelular em região central da orelha, com a confecção de retalho retroauricular ilhado transposto através de uma janela cartilaginosa e com o pedículo desepidermizado. Área doadora com fechamento primário. Tal procedimento constitui técnica segura, pois a região retroauricular é ricamente vascularizada, é de fácil execução, em único estágio e com resultado estético e funcional satisfatório.


Partial ear defects can be treated in several ways, including primary closure, healing by secondary intention, or flaps. Several surgical options have been described for reconstruction in order to maintain the natural contour of the ear, without sacrificing healthy tissues or changing the aesthetics and function. In this article, we present two cases of reconstruction of chondrocutaneous defects of the ear after resection of basal cell carcinoma in the central region of the ear, with the production of a retroauricular island flap transposed through a cartilaginous window with the de-epidermized pedicle. The donor area healed following a primary closure. This procedure can be performed in a single stage, yields satisfactory aesthetic and functional results, and is safe because the retroauricular region is richly vascularized.

SUMMARY: A detailed data of concha is currently not available. Therefore, the present study aimed to determine twelve morphometric measurements of concha, to investigate its sexual dimorphism and bilateral asymmetry, and to establish basic shapes of concha for both sexes and sides. The study sample comprised of 310 young Chinese aged 18-28 years. 141 left and 141 right ear impressions for females, 169 left and 169 right ear impressions for males were collected and scanned. The 3D coordinates of seven landmarks on each auricular concha were obtained using 3D scanning technology and curvature theory. From the landmarks, twelve morphometric measurements of concha were calculated and analyzed. The conchal morphometry exist significantly sexual dimorphism in this study sample. On average, all measurements were larger in males than in females regardless of the sides. There was significantly bilateral asymmetry among left and right conchae in both sexes. Some measurements were larger in the right sides and some measurements were larger in the left sides, but the bilateral difference in both measurements found to be less than 1mm. Additionally, the basic shapes of concha for both sexes and sides were established on the basis of the mean 3D coordinates of each landmark and the mean value of each measurement. The anthropometric method of this study could overcome the difficulty in locating landmarks of auricle complex structures, and attain a higher level of accuracy in the procedure of measurement. The quantitative description of conchal morphometry will be beneficial for plastic surgeons, and for the ergonomic design of hearing aids.


RESUMEN: Actualmente no existen datos detallados de la concha auricular. Por lo tanto, el presente estudio tuvo como objetivo determinar doce medidas morfométricas de la concha auricular, investigar su dimorfismo sexual y asimetría bilateral, y establecer formas básicas de la concha para ambos sexos y lados. La muestra del estudio estaba compuesta por 310 jóvenes chinos de 18 a 28 años de edad. Se recolectaron y escanearon 141 impresiones de la oreja izquierda y 141 de la oreja derecha en mujeres; 169 impresiones de la oreja izquierda y 169 de la oreja derecha en hombres. Las coordenadas 3D de siete hitos en cada concha auricular se obtuvieron utilizando la tecnología de exploración 3D y la teoría de la curvatura. A partir de los hitos, se calcularon y analizaron doce medidas morfométricas de concha. La morfometría conchal indica la existencia significativa de dimorfismo sexual en esta muestra. En promedio, todas las mediciones fueron mayores en los hombres que en las mujeres, independientemente de los lados. Se observó una asimetría bilateral significativa entre las conchas izquierda y derecha en ambos sexos. Algunas medidas eran mayores en el lado derecho y otras medidas eran mayores en el lado izquierdo, pero la diferencia bilateral en ambas medidas fue menor a 1 mm. Además, las formas básicas de concha para ambos sexos y lados se establecieron sobre la base de las coordenadas 3D medias de cada punto de referencia y el valor medio de cada medición. El método antropométrico de este estudio podría superar la dificultad de localizar los hitos de las estructuras del complejo auricular y lograr un mayor nivel de precisión en el procedimiento de medición. La descripción cuantitativa de la morfometría conchal será util para los cirujanos plásticos y para el diseño ergonómico de audífonos.

Abstract Background and objectives: The size of the ProSeal laryngeal mask airway in children is determined by the patient's weight. However, in some instances, an alternative method may be required. This study aimed to compare sizing by the auricle with conventional ProSeal laryngeal mask airway sizing by weight in children. Methods: After approval by the institutional ethics board and written informed consent from parents, 197 children with American Society of Anesthesiologists physical status I-II who were scheduled for a routine genitourinary operation were included in the study. The correct ProSeal laryngeal mask airway size was determined according to the size of the auricle in children. The results were compared with the standard weight-based method recommended by the manufacturer's guidelines. The patients were classified into different groups depending on the ProSeal laryngeal mask airway sizes as determined by both methods. Agreement between both techniques was evaluated with κ coefficient statistics. Results: Insertion and adequate ventilation were achieved in 185 patients at the first attempt, and 12 patients required a second attempt. Three patients had to be intubated. Agreement between the two methods of size selection of the ProSeal laryngeal mask airway was moderate using κ statistics. Conclusions: Choosing the size of the ProSeal laryngeal mask airway in children according to the auricle of the child is valid and practical. In particular, this is an alternative method in situations where the patient's weight is unknown, such as in emergency situations.


Resumo Justificativa e objetivos: O tamanho da máscara laríngea ProSeal (MLPS) em crianças é determinado com base no peso do paciente. No entanto, em alguns casos, pode ser necessário um método opcional. Este estudo teve como objetivo comparar o tamanho da MLPS convencional pela orelha e pelo peso em crianças. Métodos: Após aprovação do Comitê de Ética institucional e receber o consentimento informado assinado pelos pais, 197 crianças com estado físico ASA I-II (de acordo com a classificação da Sociedade Americana de Anestesiologistas), programadas para uma operação geniturinária de rotina, foram incluídas no estudo. O tamanho correto da MLPS foi determinado de acordo com o tamanho da orelha em crianças. Os resultados foram comparados com os do método padrão, baseado no peso, recomendado pelas diretrizes do fabricante. Os pacientes foram classificados em diferentes grupos, a depender dos tamanhos das MLPS, conforme determinado por ambos os métodos. A concordância entre as duas técnicas foi avaliada com as estatísticas do coeficiente kappa (k). Resultados: Inserção e ventilação adequada foram obtidas em 185 pacientes na primeira tentativa e 12 pacientes precisaram de uma segunda tentativa. Três pacientes precisaram ser intubados. A concordância entre os dois métodos de seleção do tamanho da MLPS foi moderada com a estatística κ. Conclusões: A escolha do tamanho da MLPS em crianças de acordo com a orelha da criança é válida e prática. Em particular, é um método opcional em situações nas quais o peso do paciente é desconhecido, como em situações de emergência.

Background: Incomplete development and growth of the pinna can lead to a small or deformed pinna, called microtia. Aim: To report and evaluate the effectiveness of a surgical technique previously described for the treatment of microtia, based on a new anatomical-surgical classification. Materials and Methods: Retrospective study of patients treated at two plastic surgery departments from January 2008 to December 2010. We recruited a total of 15 patients aged 9 to 25 years. Thirteen (87 percent) had unilateral and 2 (13 percent) bilateral microtia. All patients underwent the surgical reconstructive technique described by Firmin. Results: Among patients with unilateral microtia, seven belonged to Firmin type I, five to type II and one to type IIIa. Both patients with bilateral microtia, were classified as type IIIb. In all cases with unilateral microtia, a good initial projection of the cartilage frame was achieved during the first surgical procedure. Six patients were subjected to a second operation. During 18 months follow up, four patients (16 percent) had complications. Conclusions: This surgical technique that is based on an anatomical surgical classification, achieves favorable and satisfactory results.


Objetivos: Reportar y evaluar la efectividad de una técnica quirúrgica, previamente descrita para el tratamiento de microtias, basado en un nuevo concepto de clasificación anátomo-quirúrgica. Material y Método: Estudio retrospectivo de pacientes tratados en el Hospital Militar y Clínica Alemana de Santiago en el Servicio de Cirugía Plástica desde enero de 2008 hasta diciembre de 2010. Se reclutaron a un total de 15 pacientes, 13 (87 por ciento) de los cuales eran unilaterales y 2 (13 por ciento) bilaterales. A todos los pacientes se les realizó la reconstrucción mediante técnica quirúrgica de Firmin. Resultados: Se operó a 15 pacientes de rango de edad entre 9 y 25 años, con una mediana de 11 años. De los casos unilaterales; 7 correspondían a tipo I de Firmin, 5 casos a tipo II Firmin y un caso tipo IIIa. De los casos bilaterales, ambos tenían microtia tipo IIIb. Se logró en la totalidad de los casos unilaterales una buena proyección inicial del marco cartilaginoso durante el primer tiempo quirúrgico. Seis casos fueron sometidos a un segundo tiempo quirúrgico. Durante el seguimiento mínimo de 18 meses, se observaron complicaciones en 4 (16 por ciento) pacientes. Conclusiones: Se comprueba que mediante una técnica basada en la clasificación anátomo-quirúrgica, los resultados a corto plazo fueron favorables y satisfactorios.

ABSTRACT

El surco diagonal es un signo encontrado en el lóbulo de la oreja, que estaría relacionado con la enfermedad arterial coronaria. Nuestro objetivo fue estudiar la utilidad del signo. Se examinaron 104 pacientes (entre 30 y 80 años) clasificados por sexo y edad. Cuarenta y nueve tenían enfermedad arterial coronaria diagnosticada por coronariografía (obstrucción > del 70% en una de las grandes arterias) y/o gamagrafía de perfusión miocárdica con Talio 201 (defecto fijo). El grupo control estuvo compuesto por 55 pacientes (asintomáticos, con electrocardiograma normal). Los datos obtenidos fueron sensibilidad (61.2%), especificidad (78.2%), valor predictivo positivo de (71.4%) y valor predictivo negativo (69.3%.). Observamos una relación significativa entre la presencia de surco diagonal y enfermedad arterial coronaria. Consideramos que este signo podría resultar de utilidad en la práctica clínica, fundamentalmente para los pacientes entre 30 y 60 años.


The diagonal earlobe crease is a sign theorically related to coronary artery disease. The purpose of this study was to prove the usefulness of this sign. A total of 104 patients were examined (ages 30 to 80) grouped by age and sex. Forty nine of them were diagnosed of having coronary artery disease by coronary angiography (a 70% obstruction of one of the major arteries), and/or myocardial perfusion imaging with Thallium 201 (fixed defects). The control group included 55 patients (asymptomatic with normal electrocardiogram). Data here obtained included sensitivity (61.2%), specificity (78.2%), positive predictive value (71.4%) and negative predictive value (69.3%). We found a significant relation between the presence of the diagonal earlobe crease and coronary artery disease. We consider it a sign that could prove useful in clinical practice, mainly among patients aged between 30 and 60.

ABSTRACT

Con el objetivo de aportar datos anatómicos, que posibiliten el acceso a la región pre-auricular, con menor riesgo de lesión del nervio auriculotemporal en procedimientos quirúrgicos, fue realizada esta investigación, sobre las relaciones topográficas y biométricas de este nervio con la arteria y vena temporales superficiales y el trago. Fueron disecadas 24 hemicaras de cadáveres fijados con formaldehído al 10 por ciento, disponibles en el Laboratorio de Anatomía del Departamento de Morfología Humana de la Universidad Federal de Alagoas, Brasil. En siete casos (29,2 por ciento) el nervio subía posteriormente a los vasos temporales superficiales, y en otros siete (29,2 por ciento), subía junto a la vena. En tres casos (12,5 por ciento) el nervio emergía junto con la arteria. En dos casos (8,3 por ciento), el nervio estaba en posición intermedia. En cinco casos (20,8 por ciento), el nervio subía junto con ambos vasos. La menor distancia del nervio auriculotemporal a la línea pre-auricular fue de 0,3 mm, y la mayor fue de 11,5 mm, con media de 1,89 mm. La menor distancia del nervio con la arteria temporal superficial fue de 0,1 mm, y la mayor fue de 14,6 mm, con media de 8,66 mm, situándose la arteria anterior. Cuando la vena era anterior al nervio, la menor distancia entre sí, fue de 0,2 mm y la mayor, 10,9 mm, con media de 2,91 mm. Cuando la vena estaba en posición posterior, la distancia mínima fue de 0,1 mm, y la máxima de 9,4 mm, siendo la media de 2,54 mm. Concluimos que el nervio auriculotemporal presenta posición variable en la región pre-auricular y es la estructura más próxima a la línea pre-auricular.

ABSTRACT

O objetivo do presente trabalho foi analisar o comportamento estático da compressao de próteses auditivas retroarticulares, com AGC monocanal dos tipos "controlado pela entrada" e "controlado pela saída" em dois tamanhos de orelhas humanas, em termos de valores máximo e mínimo de NPS de saída. Seu material constituiu-se dos valores de NPS de saída de seis próteses com AGC, em duas posiçoes distintas de regulagem, nas freqüências de 500, 1000, 2000, 2997 e 4000 Hz, obtidos através de mensuraçoes na orelha externa, em doze orelhas de dois tamanhos. Na análise dos resultados obtidos, nao se observou diferenças significantes entre os valores máximo e mínimo de saída obtidos nos dois tamanhos de orelhas estudados. Observou-se, porém a efetividade das chaves de regulagem do ACG na mudança do padrao de compreessao das próteses analisadas e diferenças no comportamento das altas e baixas freqüências durante a compressao.

ABSTRACT

Se estudiaron 144 pacientes que acudieron a realizar una cronariografía a nuestro centro. De ellos el 58,2 por ciento presentó una afección coronaria. Se demostró una alta sensibilidad y especificidad entre la presencia del surco en el lóbulo de la oreja y el resultado del estudio angiográfico coronario, esta relación fue más estrecha al aumentar la bilateralidad del signo. Se relacionó el surco del lóbulo de la oreja con otros factores de riesgo, se puso de manifiesto la mayor significación de su presencia como indicador de probabilidad para padecer de una enfermedad de arteria coronaria

ABSTRACT

Two hundred term healthy neonates were examined and 14 anthropometric measurements were taken in the face and ear region. Difference between the sexes was observed in ear length, ear breadth, and philtrum. Significant correlation was found between ear placement and ear length; and outer and inner canthal distances (p less than 0.001). The paper provides baselines data for facial growth measurements and discusses the utility of these measurements in detecting dysmorphogenesis in neonates.

ABSTRACT

The Ahom has been investigated for the distribution of ABO blood groups, phenylthiocarbamide taste sensitivity, earlobe attachment and age at menarche. The present sample of Ahom displays a good amount of homogeneity with most of the neighbouring mongoloid population. Only biological features that set apart Ahom from other mongoloid populations is earlobe attachment types. It is quite possible that the higher frequency of phenotype B among the Ahom may be due to the gene flow from the surrounding caste (Indid) populations as a result of admixture.

ABSTRACT

215 patients of different communities were selected randomly to predict the correlation between the ear-lobe crease, ear-canal hair and coronary artery disease. On analysis, it was found that prevalence of the ear-lobe crease increases with advancing age, and the incidence was much more higher in Sindhis in whom the overall incidence of CAD is also significantly high. Bilateral diagonal ear-lobe crease was found to be significantly (P less than 0.001) associated in patients with documented CAD, and a significant difference was also observed between men with and without CAD in the presence of ear-canal hair with age matched group. The combined presence of ear-lobe crease and ear-canal hair was more definite and more sensitive index of underlying CAD.

ABSTRACT

El lóbulo del pabellón auricular representa, dentro de los caracteres morfológicos somatoscópicos visibles, un aspecto anatómico importante, considerando que su adherencia o no respecto de la piel de la cara implica aspectos genéticos (Recesivos lóbulo adherido; Dominantes Libre o intermedio). A objeto de comprobar las variables respecto del lóbulo auricular y su relación con las dimensiones de altura y anchura del pabellón y el índice auricular, se efectuó un análisis antropométrico en 550 individuos chilenos, de ambos sexos, de edades entre 18 y 25 años, analizando el pabellón auricular izquierdo según la norma internacional antropológica. Una vez protocolizados los datos obtenidos de las mediciones y comprobaciones somatoscópicas se realizó análisis-estadístico. Se comprueba la existencia de 37,7% de lóbulos adheridos en el sexo femenino y de 36,1% de casos similares en el sexo masculino, determinándose también variaciones antropométricas según la disposición del lóbulo auricular. Se verifica la presencia de tubérculo de Darwin o simiano en 2,5% y 1,1% respectivamente en los sexos femenino y masculino. Se comparan nuestros resultados con otras observaciones efectuadas por otros investigadores
